Patient Registration – Easy Online Submission

Joining the Electronic Health Record Sharing System (eHRSS) is entirely voluntary and free-of-charge. There are several ways for patients to register with eHRSS as healthcare recipients and online submission is one of them. Online submission is easy and convenient. Just connect to the internet, and you can do it anytime and anywhere.

Online submission is applicable to applicants who are holders of Hong Kong Identity Card aged 16 or above.

Submitting your application online is easy, and the process only takes you about 5 to 10 minutes. Simply follow the steps below.

Get your printer ready (if you wish to print a copy of your acknowledgement)

- Check the box to declare you have read, understood and agreed to the terms and conditions
   
- Input the validation code (click “Visual” and type in the characters displayed, or “Audio” and type in the characters played)

- Select communication means and provide details

- Review and confirm information input
   
- Click “Submit” to send the application (or click “Back” if you need to edit the information)

- Jot down your transaction reference number, or Click “Save” or “Print” to retain a copy of the acknowledgement

Activate your eHealth Record
 
After applying online, you can activate your ehealth record with your Hong Kong Identity Card at your next visit to a registered private healthcare provider providing healthcare to you.
 
You can also activate your record at any eHR Registration Centres in the Hospital Authority or the Department of Health, or at the eHR Registration Office.
 
Your registration will be effective afterwards and you will receive a notification of your successful registration.
