If you are aged 16 or above
You can register with the Electronic Health Record Sharing System (eHealth) online and give sharing consent to up to three participating healthcare providers (HCPs) (except the Hospital Authority (HA) and the Department of Health (DH)*). You need to complete identity verification at eHealth registration location in-person later.
If you submit your application with "iAM Smart", you can use "e-ME" to fill in your personal information (including HKIC Number, English Name, Gender and Date of Birth) to complete your registration with eHealth. Your identity will also be verified at the same time.

You can also register for the following person
For your child
You can register online for a child aged under 16 as a substitute decision maker (SDM) if you are the child's parent, family member or a person residing with him or her. You need to complete identity verification at eHealth registration location in-person later.
If your child is aged between 11-15, you can also use "iAM Smart" "e-ME" to fill in your and your child’s personal information to complete his/ her eHealth registration and identity verification at the same time.

For your family
If you and your family member are both aged 16 or above, your family member can specify on the online form to authorise you to complete identity verification for them in person afterwards.
For a person incapable of giving consent
You can register online for the following person as a SDM if you are the family member or a person residing with him or her.
- Mentally incapacitated
- Incapable of managing his or her own affairs
- Incapable of giving joining consent or sharing consent at relevant time
You need to complete identity verification at eHealth registration location in-person later.
You can also use "iAM Smart" "e-ME" to fill in your and his/ her personal information to complete registration and verify identity at the same time.

Expand- Newborn without the Hong Kong Birth Certificate. Please register in person with the newborn.
- Applications by the child's guardian, person appointed by the court to manage his / her affairs, prescribed HCP who provides or is about to provide healthcare to him / her, and the Director of Social Welfare. Please register through other means.
