If you are aged 16 or above

Please bring your Hong Kong identity card for the Electronic Health Record Sharing System (eHealth) registration at one of the following locations. If you do not hold a Hong Kong identity card, please bring along one of the required identity document.

If you are unable to submit application in person, you can authorise a person to submit application on your behalf at any registration centres in the Hospital Authority (HA) or the Department of Health (DH), or Electronic Health Record (eHR) Registration Office (RO). Please complete and specify on the Registration and Sharing Consent Form accordingly. When submitting the application, the authorised person should bring the completed form, his / her identity document, and a copy of your identity document.

For your child

Eligible substitute decision maker (SDM) can register for a child aged under 16, please go with the child and bring along the following documents with you:

  • Required identity documents of you and the child
  • If you are the child's guardian, person appointed by the court to manage his / her affairs, or prescribed healthcare provider (HCP) who provides or is about to provide healthcare to him / her, please also bring relationship proof.

For a person incapable of giving consent

Eligible SDM can register for the person incapable of giving consent, please go with the person incapable of giving consent and bring along the following documents with you:

  • Required identity documents of you and the person
  • If you are the person's guardian, person appointed by the court to manage his / her affairs, or prescribed HCP who provides or is about to provide healthcare to him / her, please also bring relationship proof.

For newborn yet to obtain HKBC

Step 1: Create an interim eHR (before obtaining the Hong Kong Birth Certificate (HKBC))

Please go with the newborn in person and bring along the following documents to the eHR Resistration Centre (RC) of the birth hospital:

  • The document issued by the birth hospital bearing unique identifier of the newborn
  • Required identity document of you

 

Step 2: Update eHR (after obtaining the HKBC)

Please bring along the following documents to any eHR RC or the Maternal and Child Health Centres of DH:

  • The registration confirmation letter showing the newborn's eHR number
  • Required identity document of you and the newborn
  • The relationship proof with the newborn e.g. birth certificate
