You may submit your Electronic Health Record Sharing System (eHealth) registration in writing by completing the Registration Consent Form and return to the drop-in box in the Electronic Health Record (eHR) Registration Office (RO)

For your child
Eligible substitute decision maker (SDM) can register for a child aged under 16. You may submit your child's registration in writing by completing the Registration Consent Form with the copy of required documents and return to eHR RO by drop-in box situated in eHR RO.

For your family
If you and your family member are both aged 16 or above, your family member can specify on Registration Consent Form to authorize you to complete identity verification for them in person afterwards.
For a person incapable of giving consent
Eligible SDM can register for the person incapable of giving consent. You may submit the person's registration in writing by completing the Regsitration Consent Form with the copy of required documents and return to eHR RO.
